An unusual shaped IUS.

چکیده

A 45-year-old nulliparous woman attended for removal and replacement of a levonorgestrel-releasing intrauterine system (IUS) for continued management of dysmenorrhoea. I had placed the IUS some 6 years previously under a cervical block, needing to dilate the tight cervical canal for insertion. The patient’s periods had been well controlled. The IUS was easily removed but, as pictured, the arms of the device were sheathed in the everted hormone cylinder (Figure 1). A new device was inserted under local anaesthesia, without need for dilation. Have any Journal readers seen something similar, I wonder?

Response regarding an unusual shaped IUS.

I read with interest Dr MacGregor’s letter regarding the unusual shape of a Mirena intrauterine system (IUS) following removal. From the photograph it would appear that the capsule has become displaced and the arms of the device enclosed within it. We have seen a case of complete detachment of the capsule from the frame, with unrecognised retention of the capsule within the uterine cavity. This...

A couple of years ago I too was alarmed as I removed an IUS through a slightly tight cervical os and noted an odd shape. On closer inspection the device was removed intact, but as in Dr Ma...

Comment on 'Unusual shaped IUS'.

shaped IUS’ Reading Dr Lloyd’s letter in the July 2013 issue of the Journal has prompted me to write in on this topic. Removal of an intrauterine device (IUD) involves traction on the threads causing the horizontal arms of the device to come together in the narrower lower half of the uterus during its downward movement. It is relatively easy for the collar of hormone in an intrauterine system (...
